It takes around a year for scars to completely mature. Until that time the scar can change. From what I can see in the pictures, there is still some redness and a raised component to the scar that may continue to improve. I would speak to your surgeon about scar management but if you are not currently using silicon gel or patches and good sun protection I would start there. Beyond those, dermabrasion or lasers all have some efficacy with camouflaging scars.

Although an examination would tell more, it is unlikely that the areas in the photograph is filler migration. It appears more likely that you are showing early signs of aging with some laxity and possibly buccal fat pad protrusion. Although injections can somewhat mask these issues I would recommend seeing a board certified facial plastic surgeon or plastic surgeon who can preform an in person exam and offer you options for treatment.

A corner lip lift is used to increase the red lip show on the corner of the lip. The traditional subnasal lip lift is great at lifting the central lip but due to the limits of where the incision can be placed, it preferentially lifts the central lip over the corners. It is very difficult to tell with the cropped pictures but 18 mm does measure on the longer side, however 5-6 mm is very short. Designing the amount to take and leave is one of the most important parts of the lip lift procedure. I typically tend towards leaving 10-14 mm depending on the age, nasal shape, lip shape, tooth show and patient desires. I would recommend setting up a consultation with a surgeon who can take good measurements and preform an assessment to let you know what is possible.
Upturned nose after rhinoplasty – Can it be brought back down to previous angle?

Depending on the rhinoplasty technique used by your surgeon the nasofacial angle will change somewhat after surgery. Many times I over rotate the angle by 5-10 degrees knowing that, as it heals and swelling reduces, it will fall into place during the first couple months after surgery. There are certain massages that your surgeon can teach you to help this settle in the post operative period. If this persists beyond 12-18 months it may require a revision procedure.
