In my opinion the best procedure you would be a face/neck lift to address the excess skin, platysmal laxity, and jowls. A skin tightening laser would provide minimal benefit for you major area of concern.

Persistent neck tightness is not incredibly common but patients have reported it with certain techniques. Without knowing the technique it is hard to pinpoint the underlying cause. Undergoing a revision to relive neck tightness could be a disappointing endeavor. The photo you posted seems reasonable and I would suggest at least trying to have a conversation with your previous surgeon
Will a lip lift pull my nostrils downwards?

No, the distance between your upper lip will shorten but the angle of the nasal tip and the position of the nostrils should remain appreciably unchanged. If your tip is plunging then that can be treated with a rhinoplasty. Thank you for the question.
Right implant still not dropping. Will I need a revision?

At this point it is likely a breast revision will be needed. Without proper examination it is hard to pinpoint the exact cause of an implant that has failed to drop. It can be capsular contracture, incomplete release of the pectorals muscle in a dual plane augmentation, fold not lowered enough, and insufficient scoring of the breast parenchyma. Pre-operatively the right side appears to be more constricted so I am sure that played a factor.

At this point far out from surgery it would most likely not be as beneficial to continue steroid shots. If you would like a different shape I would suggest consulting with someone regarding a revision.

For the lift you are looking for you would benefit from a vertical incision breast with the possibly a small “t” and a small implant to achieve upper pole volume. Achieving the volume you want in a predictable and stable manner is best accomplished with an implant.

For the best advice I would recommend that you visit a trained plastic surgeon for an in person examination. The procedure that would serve you best is lift. For the right breast you wold likely need a lollipop lift and for the left breast (smaller breast) you would probably be well served with a donut lift. At this time modest sized implants could be placed along with repeat fat grafting to any area of deficiency or contour irregularity.
