From your description it sounds like your immediate problem is capsular contracture around your implant. This will require surgery to fully remove the implant and abnormal scar tissue that has formed around it. Hi, you don’t have tubular breasts. You have more of a deflated appearance from your weight loss with some sagging. You will benefit from a breast augmentation with either a full mastopexy for maximum lifting and tightness or a circumareolar mastopexy (donut mastopexy) for some re-shaping, tightening, and slight lifting with minimal scarring. I hope this helps!

Those creases are indicative of underlying soft tissue changes as a result of aging and genetics. It probably has nothing to do with BoTox injections. At your age the best options are some type of filler treatment (Juvederm, Bellafill) to add volume and soft tissue tightening and remodeling with radiofrequency energy modalities such as Morpheus8.  There is virtually no recovery from these options and they can be done with an application of numbing cream only. From your photos it appears that your right implant is lower than your left. This may have happened after you had a capsulectomy done only on your right side. The easiest solution is to perform a limited capsulotomy on the left side and allow the implant to settle a bit lower to match the right. I assume you mean the skin is lifted up a bit on the inside edge of your breasts near the cleavage line. It will take several months for you to fully recover from BA surgery and during that time the skin will stretch out and the swelling will disappear both of which will help the skin tenting I believe you are referring to. Assuming you have healed uneventfully from your facelift and you can lay flat on your back for 2-3 hours you could probably schedule your mastopexy procedure 4-6 weeks after your facelift surgery. Personally, I do all my mastopexy procedures in my office OR under mild sedation anesthesia. Therefore, it’s a very gentle experience and won’t affect your facelift recovery.
