By limited review of the submitted photograph, I feel that the demonstrated facial vein is too large to be effectively treated by YAG laser. A better approach when the vessel is more than 2 mm in diameter involves a specialized technique of very gentle surgical removal Micro-phlebectomy. Mini vein stripping (a micro-phlebectomy) can be done through a very small incision with rapid healing and decreased risks.

Some surgical CPT codes include the procedure and the pre-op and post op care global period. Some procedures, like gallbladder surgery carry a 90 day global period. CPT code 36475 is for VNUS. 36478 is EVLT. 2 separate procedures---without a global period. Many patients can have extensive venous disease which would require multiple sessions of EVLA. If that is the case, most vein specialist would complete treatment to the major vessels of the front medial side of the limb...wait 3-6 weeks and then treat the back side of the limb.
