Statement: Dr. William A. Portuese is a double board-certified facial plastic surgeon with over three decades in practice.

He is the founder and medical director of the Seattle Facial Plastic Surgery Center and the Portland Center for Facial Plastic Surgery in Portland, Oregon.

Widely considered one of the premier facial plastic surgeons in the Pacific Northwest, Dr. Portuese specializes in all areas of aesthetic facial surgery, including facelifts, neck lifts and liposuction, blepharoplasty, rhinoplasty (including revision rhinoplasty), and forehead lifts, as well as cheek and chin implants.

“Because of my double board certification, I can extend a high level of expertise, mastery, and professionalism to my patients,” says Dr. Portuese.

Dr. Portuese earned his Bachelor of Science degree and his medical degree from Oklahoma State University. He completed an internship in categorical otolaryngology–general surgery and residency in otolaryngology–head and neck surgery and facial plastic and reconstructive surgery at Louisiana State University Hospital, followed by a year-long fellowship in facial plastic and reconstructive surgery at the renowned Lasky Clinic in Beverly Hills, California.

“Early in my career, I sought out the best of the best surgeons doing facial cosmetic surgery, specifically facelifts, eyelid lifts, and rhinoplasty,” says Dr. Portuese. “I observed and assisted these master surgeons performing their craft, and that expertise is what I bring to my practices in Seattle and Portland.”

Dr. Portuese is board-certified by both the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery.

He is a clinical instructor in otolaryngology at the University of Washington, where he teaches residents the latest techniques in cosmetic rejuvenation procedures. 50 year old man with neck laxity, particularly when looking down. Looking for advice re: options,
50 year old man with neck laxity
Nov 21, 2023
Based upon your photos, you are a candidate for a lower face and neck lift which accomplishes tightening loose facial and neck skin, tightening loose facial and neck muscles, lifting the jowls, removing the fat deposits above and below your platysma muscle in the neck, which also includes a platysma plasty to significantly improve your jawline, especially when looking down.
Facelift, fat transfer, or filler?
Facelift, fat, transfer, cheek implants or filler
Nov 21, 2023
At age 25, you certainly do not need a facelift. Fat injections are very inconsistent, we usually don't recommend them. For a temporary augmentation of the mid-face, you can consider placement of fillers in the cheeks. For a permanent solution, you can consider cheek implants performed under a brief general anesthesia.
Rhinoplasty for wide nose?
Rhinoplasty for a wide nose
Nov 21, 2023
A closed rhinoplasty approach can accomplish narrowing your nasal tip and nasal bridge line. All of the incisions are placed on the inside of the nose. An alar plasty is performed to narrow wide nostrils at rest. It's also possible to release the lateral ligaments to reduce the dynamic flare and widening when you smile. With an alar plasty, then there's a small incision at the base of the nostrils. For many examples and our current price list, please see the link below.
Do I have a deviated septum?
Do I have a deviated septum?
Nov 21, 2023
To make the diagnosis of a deviated septum, requires a thorough examination of the internal portion of the nose. A cosmetic rhinoplasty is performed to straighten the external appearance of the nose when it's crooked.
Is it possible to just finely adjust the area right below the nose?
Is it possible to adjust the area right below the nose?
Nov 21, 2023
You have what's called a hanging columella, and in particular, you probably have a prominent nasal spine. A columella plasty can be performed to trim this area back, which includes removal of extra skin and cartilage to tuck the hanging columella inwards. This can be accomplished with a closed technique with all of the incisions placed on the inside of the nose.
Should I consider a rhinoplasty revision or would a chin implant help me achieve a more balanced l
Should I consider rhinoplasty revision or a chin implant?
Nov 21, 2023
From the one limited photograph, you have an overly projecting nose and an under projecting chin. A closed rhinoplasty revision can accomplish shaving down the residual dorsal hump, and slightly de-projecting the nose. Placement of a chin implant would accomplish Augmenting your chin forward for better facial balance and proportions, especially with respect to the projection of your nose from the side profile. A revision rhinoplasty is performed under general anesthesia, while placement of a...
Can a surgeon please explain the facility fee to me?
Please explain the facility fee to me
Nov 18, 2023
The facility fee refers to the operating room where your surgery is performed. Face lifts are not performed in an office, they are performed in an ambulatory surgery center. Will I need revision? Uneven nostril, hanging columella, deviated septum
Will I need a revision rhinoplasty?
Nov 18, 2023
At this point, it's still too early to tell whether or not you're gonna need a revision rhinoplasty. Best to wait at least one year and then make that determination. From the limited photos presented, you appear to have a hanging columella. This can be improved with a columella plasty at one year after your primary surgery.
Would I be a good candidate for a lower facelift? 30 y/o female hoping to target heavy lower face
Would I be a good candidate for a lower facelift at age 30?
Nov 18, 2023
No, you're not a candidate for a facelift at age 30, since your skin tone is not an issue at that age. You have fat deposits in the neck and a recessive chin profile. Consider a neck lift with a platysma plasty and placement of a small, chin implant to Augment your recessive chin forward. Both procedures are complementary to each other to improve your neck
Sagging skin under my chin - What are my options?
Sagging skin under my chin, what are my options?
Nov 16, 2023
Congratulations on your significant weight loss. Due to the deflation of your neck tissues from the weight loss, there is now loose and inelastic skin along with platysma muscle bands present. What is the best option to fix sagging skin after neck liposuction?
What is the best option to fix sagging skin after neck liposuction at age 59
Nov 16, 2023
At age 59, you're going to have loose and inelastic skin in the neck made worse by liposuction alone. A lower face and neck lift procedure is required to tighten loose facial and neck skin in addition to tightening loose facial and neck muscles.
Candidate for Revision Rhinoplasty?
Candidate for revision rhinoplasty
Nov 15, 2023
As long as you have reasonable expectations, there is room for improvement on your nose. Just remember, it will never be perfect. The dorsal irregularities across your bridge line can be smoothed down, and the notched nostril can be improved as well with a composite graft from the ear. The attachment of your ala on the left is much higher than the right, and you were born that way with facial asymmetries. That cannot be improved. Choose your second surgeon wisely based on extensive experience.
Suggestions for non surgical lower face improvement?
Suggestions for non-surgical lower face improvement
Nov 15, 2023
To give you a better answer, it requires you posting a full set of facial and neck photographs from all angles to make a determination about how best to proceed. The non-surgical treatments only offer minimal improvement, at best.
Well, lower face and neck lift help my neck?
Would a lower face and neck lift help my neck
Nov 14, 2023
Yes, indeed, a lower face and neck lift would accomplish tightening loose, facial and neck skin, tightening loose facial and neck muscles, lifting the jowls, and removing the fat compartments in the neck located both above and below the platysma muscle in the neck itself. This also includes a platysma plasty to significantly improve your jawline. Choose your surgeon wisely, based on extensive experience producing natural results.
