Hi there, The tongue-in-groove technique is useful in certain situations in rhinoplasty but is not very commonly employed in our practice. It is helpful in our practice mainly for moving the columella (bottom part of the nose) upward. We typically do not recommend removing filler prior to a facelift as the volumes and locations do not typically create irregularities post-facelift. We would consider each patient individually but rarely would removal be necessary.

This is a very common and good question. You appear to have fullness in your lower eyelids as well as a volume deficit in the upper cheek. The most ideal result would be likely achieved with lower blepharoplasty to reduce the excess fullness in lower eyelids and simultaneous volume restoration in your mid-face. Hi there, You appear to have early looseness/laxity of your jawline and neck as well as some fullness in each area. A facelift, necklift, conservative liposuction of the neck and jowls would address the lower face. Fat Transfer would help the cheek area, and cheek implants would be another option but not my preference.  Fat could help the marionettes but a good facelift is the most beneficial for that area.Hope that helps and best wishes! Dr. Donath

Every surgeon has techniques they feel comfortable with but in our practice we find patients are most comfortable with twilight anesthesia (IV sedation). This allows the patient to have no memory of the procedure and allows us to provide the procedural details and level of precision our patients expect.

Hi there, It is difficult to tell from the profile whether you will need alar base reduction. Generally, on front view, if the nostrils are wider than the inner corner of your eyes/eyelids, you may benefit from alar base reduction. Hope that helps and best wishes! Dr. Donath

Hi there, This might seem counterintuitive but a small amount of Botox or Dysport might be the best option to achieve the appearance you are looking for, and would have the advantage of not being permanent in case you ultimately decided it wasn't exactly what you wanted. Hi there, For your particular features, you could achieve a more defined appearance of the nose and disguise the small bump with fillers but the tip definition would require adding projection to the tip (how much it sticks forward from the face) which might look too big. Hi there, you have a slightly low radix (not deep though) and so it probably would not make much difference to augment the radix. Hi there, if the bump is all that bothers you then most likely that is all you should change. We do 3D imaging in my office to help confirm for patients that they have addressed what bothers them and are more confident about proceeding. Hi there, you appear to be engaging/using the left (if first pic is reversed and 2nd and 3rd are true orientation) brow muscles more to help elevate the upper eyelid on that same side. You would need to see a doctor that specializes in eyelid ptosis and repairs of this in order to confirm.
