In my opinion you are a great candidate for what is known as an anterior neck lift. This is basically a reshaping of the muscle in the neck through 1 incision under the chin. This allows direct access to fat, including fat that is under the muscle (which you can't get to with liposuction). No skin is removed, because at your age the skin can contract and re-drape over the new shape automatically. I do not think you would have a great result with liposuction alone in my experience. ...

Definitely a rib can be removed via the closed approach no matter how it was secured. For simple removal or modification of a dorsal graft, there should be no reason to open the nose with a columellar incision.

That may be buccal fat. Excision can be done in the clinic under local anesthesia. The incision is made in the mouth and heals very well typically.

I would say until the Botox wears off, maybe 3-4 months. It probably doesn't matter that much. There is more to it than the muscle in terms of surgery. The secret to preventing drooping of the nose with smiling has more to do with tip projection. I would have to see your nose too explain more.

This is a big problem i see with open rhinoplasty, especially when rib grafts are used. It is very unusual to need rib cartilage for a primary rhinoplasty. It is possible that some of the grafts could be removed to help soften the nose. It totally depends on what types of grafts were used and what other maneuvers were performed.

You might consider a direct neck lift. This is done via an incision in the midline of the neck where the skin is excised directly from where it's loose. There are no incisions around the ears and very low risk of any complication. The scar typically heals very well. This is a great option for men with relatively isolated loose skin in the midline.

It does look like there is a subtle shadow in an inverted V formation on your upper bridge. This is caused by the cartilage in the middle 1/3 of the nose contracting in somewhat beyond where the bones can, causing a shadow to form at the lower end of the nasal bones. This give the inverted V pattern. This does not have to be fixed if it is not bothering you. If it does bother you, the best way to fix this is to cover the area with something, usually cartilage. Your laser treatments and peels should not put you at more risk for scarring during rhinoplasty, assuming the surgeon is in the correct plane when dissecting the nose. Of course, closed rhinoplasty has less chance of scarring overall and maintains blood supply better than open rhinoplasty.

Fillers will not do anything for a hanging columella. It's relatively easy to fix with surgery. On a side note, never agree to getting permanent fillers in your nose. It's WAY off label and not a good idea.
