A columella strut is used to give tip support and prevent the tip from falling so much during smiling. To obtain projection, a tip graft is placed on the domes or tip cartilage angle. Osteotomy's are the most traumatic event while performing a rhinoplasty but when necessary to give the best results should be performed.

To answer your question, yes cartilage can be taken from the left side if the right side was used and it is determined that there is not sufficient cartilage remaining on the right side. But, I think that is highly unlikely. More importantly is can your skin on your nose withstand another surgical elevation without permanently changing or hurting the skin. Only your revision rhinoplasty doctor can help you determine that.

First of all I'm a surgeon and I perform surgery for these concerns all the time. These are separate diagnosis. One is an amorphous poorly defined nasal tip secondary to spreading tip cartilages and poor tip supports. The other is flarring and elongated soft tissue nostrils or alla. The tip is addressed first. It is stabilized and defined as you desire. The nostrils are addressed conservatively because the process is not reversible. Depends on what you want aesthetically.

All noses are different and yet they can be grouped in some ways. The process is really determined by your diagnosis and what you want your nose to look like. Your nasal bridge is low and wide. It can be thinned and elevated if you want. Your tip is round, amorphous and ptotic. It can be strengthened and shaped to have more definition and not collapse when you smile. There are two areas where an upper lip lift incision can be made. One under the nose, subnasali and the other along the upper lip mucous membrane skin border called the vermillion border. The most common way it's performed today is under the nose. This accomplishes two things. One it reduces the philtrum and skin between the nose and lip and the second it produces more pink lip show on the upper lip. You are a great candidate for a subnasali lip lift procedure but be conservative.

It's all about architecture or the lack there of. The structure between your nostrils, the columella cannot hold the tip of your nose up because of weak cartilage. The tip, which is shaped like an arch gets pulled closer to the face and falls some. This is called tip ptosis and causes the tip to round out. Until your tip is supported with a columella strut, surgically the nasal tip will fall. This only gets worse with age.

You said the bridge of your nose was lowered and the breathing is great. It is not uncommon to lose tip support when the nasal dorsum is unroofed during your primary rhinoplasty. Because the tip is shaped like an arch, as it sets back, it rounds out. Sounds like you need tip support re-established. Maybe a columella strut and/or tip graft. This will give you better tip definition.

Several thoughts. Only have anesthesia if your doctor says it's required. Talk to the anesthesiologist also. You should be medically cleared by you family MD or internist. There is no basic inherent increased risk with anesthesia after rhinoplasty. Be sure your doctors know you had a recent rhinoplasty. Remind them at the time of surgery. Anesthesiologist like to put masks on patients faces and this could affect you recent nasal bone work.

This is a difficult situation because you have already had three procedures on your nose and you are not healed from your last procedure. You need to wait six more months to totally heal. Then you need an evaluation by a surgeon who regularly performs revision rhinoplasty. A correct diagnosis must be made. Then an accurate treatment plan established. Can what you want be achieved without risking your present result. Only an in person evaluation can establish that.

You have a mid nasal dorsal fullness after rhinoplasty. This is probably residual septal or upper lateral cartilage. The problem with steroid injections is that it will take down the soft tissue around the bump and base relief the fullness out. Make it more apparent. Injectable fillers above and below the bump may help or the cartilage bump may have to be resected at surgery.

Interesting question. Ptosis is specific, meaning to fall. Your chin tends to fall during smiling. The muscles are pulling it down during activation. Because you are young, a conservative approach should be considered. First consider an injection with a neuromodulator like Xeomin or Botox. This will reduce the muscle pull. The most you would consider is a conservative chin augmentation with an implant. See a surgeon who does this procedure regularly
