What are the indications for either a Deep Plan or a SMAS?

Basically, Deep plane is always the approach I use. There are few exceptions, and I have trained in all methods. Facelift 3.0 is all about releasing the deeper structures for better results.

Weight loss before surgery is a good idea, in fact it will dramatically improve your recovery and it will improve your ultimate results at the ideal weight you ultimately maintain.

Many of the options are not the best. Internal bras help lift and shape but usually they are used to push things in and up. The best solutions for your case are to recreate space in between.
Will they reschedule my surgery after medical clearance test before surgery?

It is uncommon but it happens! you definitely do not want to have an implant in the setting of infection. It could make the implant fail, requiring more surgery and making them never be as good as they could be. An implant infection could ruin your breasts!
What amount of time is reasonable for time under general anesthetic (70 year old woman)?

Anything longer dramatically increases risk of complications as we have seen in the aesthetic literature. Also If someone is taking that long to perform the surgery they may not be efficient and experienced enough for you. Consider Sedation as well though I usually do deep plane facelift in 2.5 hrs general Anaesthesia.
