There appears to be a superior malposition, with the right implant rising upward. Reoperation appears to be necessary to remove the right implant, expand the space downward, and then insert a new implant. If there is currently capsular contracture and superior malposition has occurred due to this, capsulectomy to remove the capsule may also be necessary.

To determine the size of the implant, an examination is important and various factors must be checked. I can fully understand what kind of change you want. Cartilage grafting is necessary for such changes, and diced rib cartilage grafting is believed to be best for more effective changes. I do not recommend fillers due to concerns about side effects and absorption.

Implant deviation does not seem trivial. It is obviously deviated to one side.
