Yes, marionette lines deepen as you age. The nonsurgical treatments you described are appropriate treatments, they just don’t last very long. Fat transfer and/or lower facelift are more robust procedures which last longer, although not forever. Thank you for the question. So sorry you’re experiencing the nerve problems. The answer is no. Neck Liposuction is removal of fat under the chin. The plastic surgeon is nowhere near the spine during lipo. Spinal changes are typically a result of degenerative disc disease, which is managed by a spine surgeon. I wish you all the best.

Thank you for the question. I recommend doing things in stages. Based on your photos I would suggest to start with a face and neck lift as well as a lip lift. Those procedures will go a long way to restoring a more youthful look. You can then decide if skin resurfacing and treating your eyelids would further enhance your outcome. I wish you all the best!

Recurrent Neck Bands

The recurrent neck bands are part of your platysma muscle. They can return after surgery and are frustrating to both patient and surgeon alike. As you healed from your original facelift, the neck tissues relaxed and the platysma band recurred. For now, Botox is a good approach because you’re still in the healing phase. Once your tissue has fully recovered (6-12 months post op), your surgeon may consider a second surgery to excise or suture those bands. Keep following up. All the best.

Thank you for the question. Based on your pictures, I would suggest undergoing the neck lift first. Your jowls and loose neck skin would be best addressed with a deep plane face and neck lift. That procedure alone may correct your issues thus avoiding the need for an implant. I don’t think you need an implant. All the best!

Deep nasolabial folds are challenging for all plastic surgeons. Facelifts help, but in some very adherent folds, such as yours, the skin of the fold needs to be released and volume placed under the skin. Fat would be the best option. Direct excision is an alternative, but leaves a scar. Revisit your plastic surgeon and have a discussion. All the best.

This is a challenging problem, especially after a fairly routine procedure such as lipo and platysmaplasty. It’s often a combination of organized fluid and scar tissue. An imaging study may be helpful. I would advise to continue to massage daily for now and allow this scar to fully settle and mature, as in 6-9 months, then reevaluate. It’s possible you will need a revision procedure to excise the lump and smoothen your neck. But it will be much smaller by then. Good luck.

Based on your pics I would not recommend any surgical treatment. There are some non-invasive skin tightening modalities available, such as Ulthera, but I would suggest an in-person consultation with an experienced plastic surgeon first. Good luck!

Fat transfer procedure must always come with proper expectations. You body will absorb around 10-15% of the injected fat in the first couple of months. So surgeons account for that by “over-injecting” fat by about 10-15%. Which means you will go through a phase when you think it’s been overdone. Just relax for now and allow yourself to heal. Follow up with your plastic surgeon and be reassured you are still in the early phase of healing. Good luck!

Thank you for the question. Based on your photos, I would personally recommend two things, chin enhancement with a chin implant, and lower eyelid volume, either with fat or filler. Chin surgery would strengthen your “profile view” and provide balance to the lower part of your face. And filling your lower eyelid region would restore a more youthful look. Fillers are easy, fat is more involved but probably the best option. Seek an experienced surgeon. I wish you the best!

The transconjunctival approach would be ideal for you because of your youth and the lack of excess skin. However, a transcutaneous approach with an experienced and skilled surgeon can still render excellent results. A “subcilliary incision” heals very well. The big decision will be what to do with your fat, excise or reposition? Seek out an experienced surgeon. All the best!

Thank you for the question. Based on your pics, it looks like you need a second procedure to further improve your result. Sounds like you have a caring plastic surgeon. My recommendation would be to wait a few months and allow the skin to fully heal and soften, after which a revision can be effectively performed. Thank you for the question and picture. Yes, tightening the skin with a lower face and neck lift would be the best first step. After that, there are some skin treatment options that can help further tighten your skin and increase the thickness and improve the texture. Those would be laser resurfacing and microneedling. These both stimulate new collagen formation which should n improve the appearance of your neck. Sun avoidance is a must.
