I think it might be easier to comment on your current breast shape if you were to provide pre-op photos, but I would say that your result at present is good. The flatter lower part of your breast may be a result of having had tighter tissue at the lower part of the breast before surgery which is now taking some time to fully stretch out. I would suggest that you remain patient and wait for the tissues to stretch and relax around your implants.

In my opinion the best way to determine which surgery to do in cases like yours is to define what your goals are very clearly. If you want fullness in the upper part of the breast you will need to use implants to achieve this. It sounds like you are going through a fairly normal period of healing to me. Remember that when you have implants placed the tissues need to stretch to accommodate the new implant. This includes the nerves that have now been stretched over the implant. As the nerves recover, your symptoms should get better but this can take a few weeks.

From your pictures it looks as though your nipples lie well above the inframammary fold. In these circumstances I would not perform a lift. If you have some skin on skin contact below the breast then you may do better with a subglandular lift where the implant lies above the muscle.
