Hi
The risk of developing ALCL is about 1:3000 cases. The cancer develops surrounding the implant and capsular tissue,
Current recommendations are to observe (MRI or Ultrasound) and if there is fluid surrounding the imant to aspirate and test for ALCL.
If positive then capsulectomy and explant. If negative then repeat every 2 yrs.
If you have problems with your implants such as pain, capsular contractures, BII symptoms, then capsulectomy and explant are good options.

Hi.
My advice would be to remove the implants and all capsular tissue (enbloc if possible), wait at least 6 months and then decide if you want reaugmentation.
Reasons:
1) You have had multiple complications already
2) Your inframmary folds are not even
3) You are at a high risk of developing capsular contractures in the future.

You should allow the breasts to recover and the inframmary folds to heal. Reaugmentation after 6-12 months will be easier and more predictable.

Hi.  Arnica alone or in combination with bromelain is often prescribed prior to surgery.  While bromelain has been shown to be clinically effective, the data around arnica is less convincing.We have found that perioperative supplementation with a product called Vivanta Surgical Recovery System works extremely well and has been clinically proven to reduce pain, swelling and opioid use.  Vivanta has become part of our clinics standard ERAS protocol.

Look for a surgeon that has implemented an ERAS (enhanced recovery after surgery) protocol. This includes education, standardized pre-operative and intraoperative medications, nerve blocks and perisurgical supplementation.  In our clinic we use a product called Vivanta which reduces inflammation and the need for post operative narcotics.Surgery will never be pain free, but can be quite tolerable without use of narcotics.

HelloVaser liposuction destroys the fat cells and they can not be used for fat grafting.  As Dr. Heirs mentioned, fat grafting only results in a very small augmentation.  I would wait until you are ready for a lift – that appears to be the most appropriate procedure for you.
