​Without examining you it is difficult to be certain but based on the photos it looks like the roundness in your lower abdomen extending up above the belly button indicates a separation of the muscles that can only be fixed as part of an abdominoplasty. Choosing an implant's base width needs to take into account your breast base width and the tissue thickness otherwise it will widen the breast significantly. There is generally a range of bandwidths that will fit a breast properly and this is based on measurements taken of the chest. Generally capsules start to form within a few weeks after a breast implant is placed but like all scar takes a year or so to mature/form the final capsule. For a breast implant exchange it would depend if the existing capsule was removed or not. If it was removed then its a similar timeline but in many exchanges the capsule isn't removed and will be there from the previous implant. Hope that helps!!

Without examining you, knowing your BMI or seeing photos its difficult to give specific advice but if you are concerned you are on the thinner side and wont have enough fat you can discuss a skinny BBL with a local specialist or consider something like the liquid butt lift. It's important to note that if you are going for a filler based butt lift you should use hyaluronic acid based filler rather than sculptra. Whether you need a lift to avoid sagging will depend in a large part on what implants you're getting and whether they're going above or below the muscle. Larger implants above the muscle will give more lift. That being said you do have relatively large areolae and they are at different heights, so to maximize symmetry I would also recommend the periareolar lift. Make sure your surgeon uses permanent sutures to maintain the size and shape otherwise they'll stretch out again in no time. Best...

It sounds like you've spoken with two surgeons who like to impose their opinions on patients rather than listen to what the patient wants. You haven't said what you're actually looking for here (stay the same size, go a bit bigger like the 400 or go WAY bigger like the 600). Even though an exchange is usually straightforward there are nuances to implant choice and the way the procedure is performed that can lead to better outcomes and quicker recovery so make sure to choose a breast implant revision expert, not just any local plastic surgeon.
