I would recommend that you leave matters alone and let things settle for another few months. You have a good result overall and things can take time to settle. Revision could aggravate this situation in my opinion.

I would opine that a revisional procedure with exploration of the cavity is not unreasonable. The muscle may need to be tacked down to the chest wall and reorientate anatomy. I also see a tighter lower pole of your breast and a full internal release may be helpful.
Body lift or tummy tuck?

Body lift would be the most appropriate along with liposuction for upper back folds. Body lift is comprehensive and involves tummy tuck, flank tissue removal, lateral thigh lift and buttock lift. This combination would give you the best outcome.
What other options are there for loose skin on the upper arms?

I would recommend arm liposuction and strict garment use thereafter for at least 3months. You are young and skin is able to retract well. This should do the trick for you in getting a better arm profile.

I would recommend a Z-plastic at the hooded fold to help it reorientate. This may be helpful for better aesthetic if you so desire.
Can AlloDerm or any other ADM be used to restore tissue damage below the dermis?

Difficult question to answer. My experience is in breast reconstruction where the alloderm will stick to the overlying tissue but will not bond with underlying prosthesis. It does firmly get bonded to the dermis and provides a solid layer which supports the prosthesis well.

HI in the first instance please approach your dentist and check for a normal bite. If this is normal and then the approach of balancing facial fillers both superficial and deep will work to achieve the desired outcomes. Fillers are really safe and can be done as a day procedure. This a powerful procedure to get the desired outcome.
