First, congratulations on your weight loss! That is certainly a great accomplishment.
My typical approach is to first do a lower body lift which would combine the tummy tuck and butt lift into one procedure. Usually second would be breast and arms and third would be thighs. Some of this is individualized based on people’s priorities and anatomy.

These are great questions! It is difficult to tell based on photos alone but likely with that degree of weight loss and the amount of excess skin that I can see, a full tummy tuck would probably be best. It looks as if you have excess skin above your belly button that would be removed with a full tummy tuck and not a mini tummy tuck. A full would also help to get rid of the scars and repair any rectus muscle separation.

These are good questions. If you do not have any liposuction during your tummy tuck, the only fat that will be removed is whatever is attached to the skin that is removed. We may remove some additional fat aside from that, but it is minimal.

This is a great question. There are several treatment options for a seroma. Aspiration or draining the fluid with a needle and compression is usually the first step. After this, we may recommend a drain placement by a radiologist who can then inject medications into the drain to help the seroma scar down. However, if the seroma has been there for a while, your body likely developed a capsule around it. If this does not work, surgery to remove the fluid and capsule is usually needed.

Tummy tuck candidates are individualized between patients. Usually we recommend being at a BMI of less than 30, but there are patients who are exceptions to that rule. A tummy tuck at a BMI of 32.3 would not be unreasonable if you are otherwise healthy and anatomically a good candidate. It is best to be assessed by a board certified plastic surgeon who can discuss your specific case and anatomy.

There is no harm in waiting for your exchange surgery. Depending on the type of cancer, there are patients who get radiation to their expanders who then wait 6 months to get the exchange. The biggest risk is rupture of the expander but this is uncommon. It is best to discuss this with your board certified plastic surgeon.

These are great questions! Breast reconstruction is highly individualized and dependent on your health conditions, size and shape of breasts, excess skin/fat in other areas of the body, and many other things.
With regards to implant reconstruction, putting implants above the muscle is definitely the gold standard now. You can discuss going through a traditional 2 stage reconstruction versus going direct to implant as well depending on your breast size and shape.
