The best tools in my tool box for peri-oral wrinkles is to soften the wrinkle-making muscle with Botox and resurface with CO2 laser. Yes it has the longest downtime, but high energy=longer downtime=better results.

You look like you have been taking care of your skin-I can’t see the sun damage you refer to, so–keep it up. Use good products! Make sure you also prevent deepening wrinkles with regular Botox sessions–they work!You would benefit from temple, nasolabial fold, and maybe a little (natural) lip filler placement to add more youthful looking volume. I always recommend starting with baby steps-Botox and fillers are easy, low risk way to maintain (or achieve) a more youthful appearance.
Should I do Bichat’s fat pads or chin lipolysis first if I do them in separate clinics?
Buccal fat pad reduction and liposuction

I recommend (and do) buccal fat pad reduction with submental liposuction often. It makes since as you will already be in the procedure room and the areas will recover at the same time. I often will do these with a chin implant at the same time.Go for it!
How to fix asymmetrical face? Had buccal fat removal when I was 21.
Filler, Fat, or Renuva

You certainly can have fat grafting to replace the asymmetrically removed fat. Yes, it may take more than one session as not all the fat transferred survives the transfer process. Another option is a new product called Renuva which is an injectable composed of adipose allograft matrix with collagen, proteins, and growth factors derived from donated and purified fat. This stimulates your body’s stem cells to make more of your own fat. A bit pricey, but–no surgery.
Please advise if I should consider buccal fat removal or an alternative procedure?

Typically, buccal fat pad reduction is recommended in younger persons. As we age we lose volume in our faces. I tell all my buccal fat pad reduction patients that it will look good in your 20s and 30s, and you will probably benefit from strategic fat transfer back into areas of the face in your 40s and 50s+.It isn’t a big deal for you from a surgical standpoint-short, minimal pain, risk, recovery- but you might “miss” the fat in a few years.
