Best non-invasive initial treatment is with Coolsculpting because there isn’t as much downtime (i.e. bruising and swelling) as with Kybella. I like Kybella for touch ups of smaller areas after Coolsculpting.

You are correct that filler to upper cheeks can make the lower face appear thinner by comparison. Please see an expert to make sure not overdone. Hard to tell from picture, but if any volume needed for medial cheeks, using a cannula would be safer. Face shaping can also be accomplished with Botox to shrink masseter muscles and surgical removal of buccal fat pad though could see if filler would be enough first.

Vollure and Volbella are my preferences for filling lips. They are cross-linked hyaluronic acid fillers. The cross-linking helps prevent them from being broken down as fast and keeps them from swelling as much as other hyaluronic acid fillers.

Voluma to your cheeks is needed more. But, why not both? With a blunt-tipped cannula through one injection point, you could also fan a little Voluma to your nasolabial folds after getting the medial cheek. I would put probably put 40% of one syringe of Voluma to each cheek and 10% to each nasolabial fold.

Studies on microneedling have shown that the holes created in the skin take about 4 hours to close. So, following injection of fillers, I recommend my patients not applying anything to their skin for 4 hours and that would include a spray tan. Also, don’t like the idea of having spray tan particles on the top of the skin prior to injections that could be pushed under the skin with filler. Maybe you could save money for filler by not doing spray tans? I wish their wasn’t such an…

Nasolabial folds and marionette lines can respond well to fillers. Even though you have already had 3 syringes, it does appear you could benefit from more. Proper placement is incredibly important so you can get the most from each syringe of filler. Due to the depth of your folds, it may be hard to completely erase them. But, Vollure can work well here.
