Statement: Dr. Paul K. Holden is a board-certified head and neck surgeon in Scottsdale, Arizona. He is highly skilled in all areas of plastic and revisional surgery concerning the face, neck, nose, chin, and ears, with a primary emphasis on rhinoplasty and face and neck lifts.

Dr. Holden’s artistic talents and innovative techniques in the field of facial aesthetics led him to develop his endoscopic ponytail upper facelift, which lifts the upper brow area, eliminating loose skin, for a smooth and youthful appearance. What makes this procedure especially impressive is the use of small incisions along the hairline, resulting in no visible scarring.

After graduating from the University of California with a masters in biochemistry, Dr. Holden went on to earn his medical degree at the Keck School of Medicine of the University of Southern California, where he graduated in the top 5% of his class. He attended UC Irvine School of Medicine to complete his internship in general surgery and residency in head and neck surgery. He was fellowship trained in Facial Plastic and Reconstructive under the renowned plastic surgeon, Dr. Stephen W. Perkins.

A recognized expert in the field of facial cosmetic surgery, Dr. Holden’s has authored several textbook chapters on facial plastic surgery and his research—particularly in the areas of cutting-edge techniques in revision rhinoplasty, eyelid surgery, and Botox—has been published in numerous medical journals. He has served as a clinical instructor on the faculty at the University of California and Indiana University, schooling medical students and residents in the art and science of facial plastics and head and neck surgery.

Bad septorhinoplasty. What should I do?
Undesired results
Mar 13, 2018
Hello cyber.girl19,
I am very sorry for your experience. Rhinoplasty is an art and can be very technically demanding. Additionally, it is important that you and your surgeon communicate well.
As it appears now, your nose is a lot more complex than before your first surgery. The good news is that it CAN be improved. Proceed with caution and select a surgeon that performs a lot of revision rhinoplasty and has a good reputation.
As far as snoring goes, this...
Is there a cosmetic surgeon in the Scottsdale, AZ area that will provide a special rate to receive b
Combo surgery in Scottsdale
Mar 7, 2018
Our practice is in Scottsdale and we do perform both surgeries under one anesthesia. It is important that you select two specialists, one who performs a lot of rhinoplasty and one who performs a lot of breast surgery. The skills involved in those two operations are very different. having both surgeries at one time does not prolong recovery time and is a sensible decision. I hope this answers your question. I wish you all the best in your journey.
Completely transforming my nose and lips: I am 18 years old Asian guy.
Augmentation Rhinoplasty and Lip Reduction
Feb 4, 2018
The best treatment for what you desire is called an augmentation rhinoplasty. This involves using either cartilage or an implant to increase the size of your nose (on profile as well as the tip). There are many pro's and con's of using cartilage or an implant. Additionally, if you choose cartilage, there are several options as to the source of that material. It is important that you consider all options with your surgeon during consultation.
With respect to lip reduction,...
Do I need a facelift already at 29 years old?
Options besides a facelift
Feb 4, 2018
Thanks for your question and the photos. Based on your history of having some children and the photos, the laxity you are experiencing is not entirely surprising. Still, I believe it would be too much surgery to undergo a facelift or neck lift to address your concern. You have a petite jaw, which is partially to blame for your situation as well. A chin implant in addition to a submentoplasty (tightening of the muscles under your chin) with liposuction of that area...
Saggy face at 42 with deep marionette lines, jowls, NL folds, and turkey neck. What is the best solu
Your neck and jowls can be fixed!
Feb 4, 2018
Great question and thank you for your photos. As I'm sure you could guess, the solution to the loose skin and muscle around your lower face and neck is to lift and tighten those tissues. The procedure, done properly, should address your concerns. Most notably, the jowls and neck will be helped the most. Nasolabial folds sometimes require additional procedures or injectable fillers, but the vast majority of patients with your particular set of issues are happy after a...
Are drains placed after a face and neck lift? And for how long?
Drains are usually not necessary
Feb 4, 2018
Thanks for your question. In my experience, which hundreds of facelifts, drains are rarely needed. I use a natural tissue adhesive and coagulation promoter at the time of the operation as well as a properly fitting dressing. In the cases where the patient has more bleeding than usual (usually due to their genetics or other health factors) I will place small drains for 24 hours. This happens about once every 18-24 months in my practice, and is well tolerated by the...
I am considering lower facelift for sagging jowls. Worth it? Will jowls be gone?
A facelift is the best bet
Feb 4, 2018
Thank you for your question and photos. As you guessed, a facelift would best address the jowls and jawline area. This is because the cause of your issue is laxity and the best treatment is to lift and tighten laxity. Less invasive options like using ultrasound or radio frequency ablation would be costly, painful and would almost certainly disappoint you. Additionally, that particular area has some nerves that could be injured with those procedures.
The good...
What is the best option to reshape my nose?
Rhinoplasty is your answer
Feb 4, 2018
Thank you for your question. Your photos appear to agree with your assessment - the tip is bulbous and the profile is tall and convex. I also notice that the nose is wider at the base. Based on the noses you like, I suggest that a rhinoplasty be performed with the goal of refining the tip, lowering the profile (side view), narrow the nose (especially the base) and raise or stabilize the tip a bit. This would give you a very nice result. The recovery,...
What can be done to make my nose and nostrils more symmetrical?
Bringing symmetry to your nose
Feb 4, 2018
Thanks for your question. The good news is that deviated noses and asymmetric nostrils can be improved. Of course, there is no substitute for an exam, but based on your photos, I believe your goal is reasonable. A rhinoplasty would address the deviated nose and also bring some added refinement to the tip and overall width of the nose. At the same time, nostril narrowing could be done to both reduce your nostrils and make them more even. An experienced surgeon...
Looking for advice on how to have a more attractive face
Add harmony to your face
Feb 4, 2018
Thank you for your post. The photos, to me, demonstrate a few areas that would help you reach your goal. Firstly, I agree with you that a rhinoplasty that removes the convex part of the bridge as well as refines and thins the nose would be a great start. Your chin appears to be small (under projected) in the photos, so a properly sized chin implant would bring some great balance. Your lower eyelids would benefit from a transconjunctival blepharoplasty (the...
My nose is very swollen 1 week post op. Will it go down significantly?
1 week is way to soon to tell
Dec 28, 2016
Hi shope,

Any surgery, even "just" a tip revision, will cause swelling in your nose that can take up to a year to resolve. In many cases, much of the expected definition will be visible in the first 3-6 months, but it is not a reasonable expectation that it will not be swollen after a week.

Things you can do to help prevent this type of swelling are: 1) Consume a Low Sodium Diet 2) Sleep with your head elevated 3) Avoid sun or any injury (like squeezing...

Can you tell me what was done here? Was the front of my septum or columella cut to be made shorter?
No change if that was all that was done
Nov 3, 2016
The short statement you have given is describing the harvesting of cartilage from deep inside the nose and taking care not to affect the support. If that was all the surgeon performed, it should not cause a change in the tip rotation or columella at all.

The posterior (back or deep) septum and superior (higher up) septum are fine to harvest, provided at least a 1cm wide "L" shaped part is still present along the bridge and down the columella.

You really should discuss...

I had a revision rhinoplasty 4 years ago, and still have a big tip and droopy nose. I am very upset.
Improvement Is Possible
Oct 18, 2016
Hi Val0808,

Thanks for your question and related photos. Based on your short history and what I can see, I believe your nose can be improved upon. It is normal to feel that it may be hopeless after you have gone through more than one surgery and did not get the results you desire. However, in the hands of a surgeon who does a lot of complex secondary (revision) rhinoplasty, your chances are very good. Because I do a lot of this type of corrective work, I am comfortable...

My footplates were cut off by my doctor. Can anything be done to replace the little triangle?
Structural Revision Rhinoplasty
Sep 25, 2016
Thank you for your question. I am sorry you are going through this. In revisions requiring major tip reconstruction including adding length and columellar support, it is likely you will need cartilage grafting from a source besides your nose. This would either be ear (most likely) or rib. In that case, the grafts might be able to be carved in such a way as to re-create damaged or lost structures like the medial crural footplates. When doing this, I always...
Can this lazy eye be fixed? What are the risks?
Yes, it can be fixed
Sep 25, 2016
Hello millyjhud,

Your photo shows moderate to severe upper eyelid ptosis of BOTH eyes, though your left eye looks a bit more severe than your right. Although not all, many surgeons that are in other specialties (like otolaryngology, facial plastic surgery and plastic surgery) can and do perform the operation to repair this condition. In my training and practice this is a frequent occurrence. It is very common for patients who have had this condition since birth to have it...
