Due to his extensive experience with both the departments of Plastic and Reconstructive Surgery and the Department of Neurosurgery at Johns Hopkins, Dr. Santiago is the world’s first surgeon trained in the specialty of Neuroplastic and Reconstructive Surgery. He has performed more cranial reconstructive surgeries than most plastic surgeons perform in a lifetime and often provides consultations to patients, neurologists, and neurosurgeons.

How much has cartilage of septum recovered after 5 months post septo rhinoplasty?

The cartilage of the septum actually does not grow back after septoplasty. As long as adequate amount of cartilage was left during the procedure to provide support to our nose (prevent collapse), there should be no concern about the missing cartilage from the septum.

I would say someone looking at you would think you have a very nice result. If you are unhappy with the appearance however, lowering of your nasal tip in order to show less nostril is a possibility. Oil in the nose should not affect healing in any way. Nasal saline spray is best to clean the nose if any unintended substances end up inside.

It appears the nose is deviated and sometimes the use of taping the nose back into position while the healing process is still ongoing will allow the cartilage to straighten again (this should be done by your surgeon). The majority of structures in the nose are held together with suture. Sometimes small free cartilage or fascia grafts are placed in an on-lay fashion and can move if you touch your nose too much. Small things touching your nose shouldn’t affect graft position or sutures. Forceful trauma in the post-op period however can cause damage requiring correction.

Do you have splints in your nose with breathing holes? If so, keeping your nasal cavities moist with nasal saline sprays will prevent blood and mucous from drying and obstructing your nasal airway. Sometimes if patients have clogged nasal splits I’ll have them come in to suction their nasal cavities of dried mucous and blood to improve air flow.

Your thought process is right on, however its difficult to answer your question regarding the plastic surgeon’s plan without a photo. In general, nasal tip cartilage can also be manipulated without addressing the radix or dosrum to achieve a supra-tip break as well. The plan may include a bit of both.

A perforated septum could result in collapse of the dorsum. it’s hard to tell from the picture you posted because it’s taken from a high-angle, however if you lost cartilaginous support, it could result in a ‘dip’ appearance over the area of collapse. It appears you still have a fair amount of swelling which can be expected at only 2 months after surgery. If your concern is about nostril size, the membranous septum /columella could be narrowed however I would wait several months before considering any type of revision procedure. The swelling may go down enough that you end up being happy with the results.

It does appear that some bone or calcified scar tissue is causing the bump on the radix. This could be corrected during a revision surgery. I don’t think it would make your nose appear shorter as your nasal starting point would remain in a similar position.
