Based on this photo, you definitely have left upper eyelid ptosis, and depending on whether you exhibit "hering's phenomenon", you may also have right upper eyelid ptosis that is currently being masked. Fixing the left eyelid ptosis, may unmask the ptosis on the right eye.
You definitely need evaluation by an ASOPRS trained Oculoplastics surgeon.

Technically, removal of extra skin and re-creating a crease is called and "anchor blepharoplasty". Some patients may also need fat excision as part of their blepharoplasty, though we tend to be very conservative with fat excision to preserve volume.
The levator resection (or advancement) is the ptosis repair part of your surgical plan.
So if you surgeon is planning on skin excision, with levator advancement ptosis repair, plus crease reformation, then you are having both an anchor...

Based on your posted photo, you do in fact have eyelid ptosis, or droop. The left eyelid looks droopier than the right, but I suspect that if you treated just the left side, the right side will relax and require surgery as well. There are a couple different techniques that we use for ptosis repair, external levator advancement vs internal muller's muscle shortening. Based on my examination, I will recommend one or the other, as each has its pros and cons to it.

IPL is an effective and safe treatment when performed by a skilled, licensed technician along with following proper pre- and post-treatment instructions. Another good option would be the Long Pulse Nd:YAG 1064nm laser, which is used for treating those small, broken capillaries.

Hard to give a diagnosis without an examination, but if it is worsening, then you should see an Oculofacial Plastic Surgeon (Oculoplastics). Since you are in Vancouver, the Casey Eye Institute at OHSU would be a good place to start.

Based on your first photo, you have mild right upper eyelid ptosis (levator disinsertion) with right brow elevation as a compensatory mechanism. This also leads to multiple crease formation because your levator is not properly pulling your crease inward.

A more predictable and long lasting solution would be a hard palate graft to elevate the eyelids. You can augment results by adding an orbital rim implant. That would increase complexity and possible complications....high risk but high reward.

You have mild upper eyelid ptosis that can make your eyes appear smaller. Surgery is the definite treatment, but there is a new FDA approved eye drop called Upneeq which does lift the eyelids temporarily. I do not believe it is a good long lasting solution, but you may want to try it.

The internal ptosis repair method involves shortening the conjunctiva and muller's muscle. If you still have enough redundant tissue, it may be possible to repeat the procedure, but if there is not enough tissue and it is attempted, it could lead to scarring of the eyelid to the eyeball and possible cause poor eyelid closure or even double vision.
Choose a surgeon that has done this previously and is experienced enough with the required examination to identify if you are a good candidate.
