Can I Use Botox if I Have Glaucoma (Open Angle?)
Botox CAN be dangerous for people with Angle-Closure Glaucoma
Jan 23, 2023
Thank you for your question, and in contrast to all of the other comments here, Botulinum toxin CAN be dangerous for people with Angle-Closure Glaucoma.

Botulinum toxin administered around the eyes for any reason (e.g. cosmetic procedures, blepharospasm, etc.) can cause temporary pupil dilation which in turn may lead to acute angle closure in high-risk populations. The pupil dilation may be present for 3 weeks and then disappears gradually.

Botulinum toxin can also lead to a temporary...

Will I have issues closing my mouth after a lip lift?
Lip Lift in the setting of Lip Incompetence / inability to easily bring my lips together
Jul 26, 2022
You may have what’s called lip incompetence. One cause of lip incompetence is a dental protrusion, such as an overbite, overjet, or buck teeth.

A lip lift done correctly should not in and of itself cause issues closing your lips.

But in your case, I would refer you to one of my dentists for an evaluation prior to the lip lift. We work in tandem to give our patients the best care.

How much would it cost to repair my earlobe? Can I keep my gauges after surgery?
Modern Earlobe Lift for a Torn Gauged Earlobes
Jul 26, 2022
Earlobe surgery is delicate and I’ve developed and pioneered the Modern Earlobe Lift.

You may be able to keep your earlobe gauged but this would be determined after an in-person evaluation.

Is a lower blepharoplasty my only option?
Undereye Weekend Lift for deep tear troughs
Jul 26, 2022
This is a problem other young individuals have and it’s essentially the outline of the ligamentous attachments and your orbital rim.

You’ve had dermal fillers before but that doesn’t mean they’ve been done properly.

My Undereye Weekend Lift is a non-surgical procedure I’ve developed to help issues like yours.

Do I need brow bone surgery or eye lid surgery?
Eyelid Retraction
Jul 26, 2022
You appear to have upper and lower eyelid retraction based on these photos, and one medical condition you would want to evaluate for is Thyroid Eye Disease.

See an Oculoplastic surgeon for a proper evaluation and recommendations.

Why aren't the apples of the cheeks lifted vertically during a facelift?
Lifting the Apples of the Cheeks
Jul 26, 2022
A deep-plane facelift with ligamentous release and malar elevation should lift the apples of the cheeks.

Facelifts should have a vertical component and there are different opinions on the overall vector.

I performed an extended deep-plane facelift with ligamentous release and malar elevation when indicated.

Seriously considering eyelid surgery. Will it fix my issue or should I consider other better
Undereye Weekend Lift for Lower Eyelid / Undereye Wrinkles
Jul 26, 2022
Difficult to tell what’s going on based on your photos and the fact that you’re wearing glasses.

Lower blepharoplasty is good for fat bag prolapse.

Wrinkles can be addressed in other ways. My Undereye Weekend Lift is a non-surgical procedure that can help with not only wrinkles, but also fat bag prolapse (if present) in many patients.

See an Oculoplastic surgeon for a proper evaluation and recommendations.

Why does my left eye have a bulge underneath it after lower blepharoplasty?
Revision Blepharoplasty
Jul 26, 2022
It’s difficult to see the bulge you are referring to and to tell what’s fully going on, but you appear to have lower eyelid retraction, scarring, and abnormal eye shape.

Unfortunately, you may need another revision surgery.

See an Oculoplastic surgeon who is experienced in revision blepharoplasty for a proper evaluation and recommendations.

What will correct my under eye hollowness?
Undereye Fat Bags, Hollows, and Flat Midface
Jul 26, 2022
You appear to have prolapsed lower fat bags, a deep tear trough, and a flat mid-cheek. This combination is likely making you feel you have severe undereye hollows.

A Lower eyelid blepharoplasty utilizing a hidden incision approach through the inside of the eyelid with fat repositioning can help address your issues.

I would also consider facial fat transfer. As the Course Director for the Academy’s Fat Grafting course for many years, there are nuances in fat transfer and it has to be d...

Age 40 & at a loss for deep tear troughs - what can I do?
Deep Tear Troughs
Jul 26, 2022
The photos are not enough to determine what’s going on, but you may have lower eyelid fat bags and edema, malar edema, skin laxity, flat cheeks, and possibly a negative vector eye morphology. You also have a strong demarcation of the tear-trough and this likely represents the ligamentous attachments and may be exacerbated in appearance because of the flat cheeks and eyelid edema.

I would start with thoroughly dissolving all the filler and evaluating you for various medical conditions t...

I have droopy eyelids. What eyelid surgery could help me?
Surgery for a droopy eyelid
Jun 27, 2022
You’re not looking straight into the camera, but it appears you have one droopy eyelid. It could certainly be both eyelids. But you need a ptosis evaluation to see why you have ptosis in the first place. There are many different reasons for having ptosis, whether it’s on one side or both.

There are different surgical approach options for ptosis and the one I recommend will depend on the case. But I more commonly perform the scarless approach from the undersurface of the eyelid.

I’m an Oc...

What can I do for under eye wrinkles/bags/loose skin/malar edema?
Long journey of fillers, dissolving fillers, energy device, Botox
Jun 27, 2022
You’re smiling in all your photos to it’s difficult to tell the state of your undereyes.

You’ve been through a long and drawn-out journey and there are a lot of questions I have which can’t obviously be answered on this platform.

But a couple of things:
- lower eyelid edema can be an indicator of a medical condition and not necessarily something “cosmetic”
- be cautious in signing up for a lower blepharoplasty - you may not need it and it could make things worse.

See an Oculoplasti...

Bruised and swollen lower eye bags 3 months after eyelid surgery - what can I do to resolve i
Scars after lower blepharoplasty done through the eyelid skin and not transconjunctival
Jun 27, 2022
Once reason I perform lower blepharoplasty through the transconjunctival (inside of the eyelid) approach is to avoid these scars. However it’s a more sophisticated technique that many don’t know how to do well.

I typically like to start scar injections early on at 3 weeks after surgeon when needed, although they have their own risks and they are off-label injections in the US.

You also shouldn’t have bruising and swelling 3 months after surgery.

You need a proper in-person evalu...

Unhappy with the loose skin under eyes - is lower blepharoplasty the best treatment for me? (
You need the Undereye Weekend Lift
Jun 27, 2022
The Undereye Weekend Lift is a procedure I’ve pioneered to address much of the issues you’re wanting to address.

It will help the hollows, skin texture, and wrinkles when smiling.

A lower blepharoplasty would be helpful if you had lower eyelid fat bags and lower eyelid skin removal would be helpful if you had a lot of skin redundancy. So it seems those options are off the table.

I’ve attached a video showing a patient who opted for the Undereye Weekend Lift and she in fact was a candi...

At 54 years old, when is it best to get a facelift for the best effects?
Best age to get a facelift
Jun 27, 2022
Interestingly enough, I don’t look at how young a patient is, but actually the opposite, I look at how old a patient is. Patients on the other end of the age spectrum have more medical problems, a more difficult time with surgery, and a slower healing time. So there’s more to consider.

For people to not notice you’ve done anything means you want a natural result. And a natural result does not necessarily mean a non-surgical or minimally-invasive procedure.

You want the correct proce...
