After earning his medical degree from the College of Medicine at the University of Vermont in Burlington, Dr. Darrow completed a general surgery residency at the University of Massachusetts in Amherst, a plastic surgery residency at University of Southern California, Los Angeles, and a plastic surgery fellowship at the Rancho Los Amigos Medical Center in Los Angeles. He has been in private practice performing cosmetic plastic surgery since 1994.

Your surgeon should always have full knowledge of all prior procedures that you have had done. Particularly if the surgery is in the area where they are going to operate. That said, repair of a hiatel hernia would either be done laparoscopically with several small puncture wounds, or would be done open, with a vertical incision. Either way should not interrupt the blood supply to your abdominal skin enough to cause any problems and it should be safe to proceed. Best wishes.

As the cause of your concern is over resection of the fat underneath your skin, which then exposed, underlying muscle and jawline, and gave you a harsh appearance to your neck, the only treatment that I did not see that you tried but may be of some help would be fat grafting to try and add volume to soften the appearance. In order to smooth your jowl, and straighten your jawline, the fascia layer of your face, the SMAS, would need to be elevated in a vertical fashion, to elevate the jowl off of your jawline and efface it. This could be done through a mini facelift. A simple skin lift would likely not have long lasting results as the skin is not a support structure. It is a covering. Asking that skin to be a support structure leads to early recurrence of the jowls. Best wishes.

An upper body lift should easily be able to be performed as a single procedure so that you have one operation and one recovery. This assumes, however, that you have the proper facility to do it in and proper anesthesia and the surgeon can complete the procedure in 6 hours or less. After removal of your excess skin, you could request your surgeon use a deep layer of sutures plus staples, which you said have given you a non complicated, well healed wound in the past. There is no requirement to use Dermabond or any skin adhesive. This should alleviate the rashes and reactions that you were having. Antibiotics would not help prevent or treat the reaction you are having to the Dermabond as it appears to be an allergic reaction. Best wishes.

A breast lift is done by removing some of the skin envelope of the breast, but maintaining all of the underlying breast tissue and shifting it to a higher position, and wrapping the remaining breast skin envelope around the tissues. If your breast skin stretches during future pregnancy but does not “snap back”, additional tightening of the skin can be performed. The same pattern would be used for both breast lifts, and therefore it would not increase your scarring. Best wishes.
